Hearts With A Mission in Oregon is seeking a Health-Related Social Needs Case Manager (HRSN CM) for a full-time position in Klamath County. The role focuses on screening, intake, and navigation to connect community members with essential social services.

The candidate will document data, verify eligibility, and collaborate with care teams, schools, and organizations. A Bachelor’s degree in a related field and 2+ years of experience with at-risk clients are required; a driving license is needed.

Qualifications

• Bachelor’s Degree in Education, Social Work, Psychology, or a related field is required.
• Minimum of two years’ experience working with at-risk clients.
• Valid Oregon driver’s license at time of hire.
• Knowledge of DHS Mandatory Reporting and community resources.

Responsibilities

• Screening and intake assessments to determine eligibility for HRSN services.
• Coordinate referrals and navigation between clients, care teams, and partners.
• Enter and maintain data in databases with privacy compliance.
• Review eligibility documentation to facilitate timely service delivery.
• Collaborate with interdisciplinary care teams to align service plans.
• Monitor requests and follow up to connect resources.
