Job summary

Innovative Integrated Health Inc. (PACE by IIH) seeks a Health Records Clerk to ensure accurate participant records within our home-based elder care program.

Under the supervision of the Health Records Supervisor, you will prepare charts, scan documents, and maintain records while collaborating with clinic, scheduling, intake, and interdisciplinary teams to coordinate care. Strong MS Office and EHR skills are required.

Qualifications

• Minimum of one year experience in Health Records.
• Experience working with a frail or elderly population.
• High school diploma or equivalent; Medical Assistant Certification preferred.
• Medical Terminology certificate preferred.

Responsibilities

• Prepares charts for new participants in collaboration with intake department.
• Scans documents into participants record daily.
• Maintains participant records by collecting test results, specialty consults, procedure results and other data.
• Collaborates with interdisciplinary team in participant care planning.
• Routes notes to ordering provider for review and care coordination.
• Adheres to IIH policies and maintains confidentiality.
