myPlace Health is a mission-driven PACE organization focused on helping older adults live safely and with dignity in their communities. We seek a Medical Records Specialist to manage records securely, collaborate with healthcare teams, and keep processes running smoothly behind the scenes.

The role emphasizes accuracy, regulatory compliance, and teamwork. On-site location supports a structured, supportive environment with opportunities to contribute ideas and impact participant care.

Qualifications

• Strong communication and interpersonal skills to interact with healthcare teams and participants.
• Attention to detail to ensure accurate medical records and documentation.
• Comfort with HIPAA and regulatory requirements guiding medical records handling.

Responsibilities

• Secure and maintain signed medical record consent forms and store in EMR or paper files.
• Process incoming faxes, emails, and mail containing medical records efficiently.
• Handle urgent medical record requests in collaboration with the IDT and Compliance.
• Retrieve and release medical records to authorized providers in compliance with HIPAA.
• Review records for accuracy and ensure documentation meets DHCS and CMS standards.
• Create participant charts in the EMR and keep records up to date.
• Coordinate with external vendors for medical records while meeting regulatory guidelines.
• Provide general administrative support including scheduling as needed.
• Collaborate with Risk Adjustment and Enrollment teams to ensure proper documentation.
• Support the team with additional tasks and AI-related initiatives.
