Job summary

Cook County Government seeks a Community Resource Navigator for Health Plan Services / Managed Care - COUNTYCARE. The role supports care coordination for members in homes, clinics, and hospitals, helping navigate the health system and reduce barriers to access.

Responsibilities include risk screenings, member education, appointment scheduling, referrals, and collaboration with care coordinators to ensure timely care and optimal outcomes.

Qualifications

• Excellent verbal and written communication with diverse populations.
• Ability to track meetings, appointments and emails in Outlook.
• Ability to prioritize work and meet deadlines.
• Employee must have a valid IL driver’s license and vehicle insurance.

Responsibilities

• Complete health risk screenings and document responses.
• Educate members on navigating the health system and accessing services.
• Schedule appointments and refer to community resources; follow up to close care gaps.
• Assist with applications for services (DRS, DOA, HDM) and mail confirmations to support coordination.
• Link members to appropriate programs under supervision of CBSWCC.
