ATW Health Solutions is seeking a Community Health Navigator to connect residents in Cook County with Medicaid redetermination and enrollment services. This contractor role emphasizes in-person engagement at community events, offering one-on-one guidance and helping people navigate Cook County Health resources.

The ideal candidate will be organized, culturally aware, and able to work evenings and weekends with reliable transportation.

Qualifications

• High school diploma or GED required.
• 1–2 years of experience in community outreach or healthcare navigation.
• Strong interpersonal and communication skills with cultural humility.
• Willingness to work evenings and weekends and travel across Cook County.
• Reliable transportation and ability to use state enrollment websites.

Responsibilities

• Educate community members about Medicaid redetermination timelines and requirements.
• Assist individuals in completing Medicaid redetermination forms and gathering documentation.
• Serve as a liaison between residents and Cook County Health services.
• Provide in-person guidance to navigate available resources.
• Attend outreach events across Cook County, including evenings and weekends.
• Build relationships with local partners such as churches and shelters.
• Distribute educational materials and provide enrollment support onsite.
• Maintain records of community interactions using simple tracking tools.
• Participate in team meetings and quality improvement efforts.
• Follow onboarding procedures including check-in and follow-up communications.
