Job summary

Sweetwater Care is seeking a Medical Records Director (HIM) to oversee accurate, timely, and confidential maintenance of resident health information in compliance with Colorado state regulations and federal requirements. This role supports clinical operations, survey readiness, and record integrity within our skilled nursing facility.

Essential duties include maintaining and securing records, ensuring chart accuracy, coordinating health information releases, and supporting audits and QAPI

Qualifications

• High school diploma or equivalent required; HIM coursework preferred.
• Minimum of 1 year of experience in a skilled nursing or long-term care setting preferred.
• Proficiency in EHR systems and Microsoft Office applications.
• Strong organizational skills, attention to detail, and ability to meet regulatory deadlines.
• Excellent verbal and written communication skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Maintain, organize, and secure medical records in compliance with state and federal regulations.
• Review medical charts for accuracy; follow up with staff to resolve deficiencies.
• Ensure documentation meets survey readiness standards at all times.
• Coordinate release of health information per HIPAA and privacy laws.
• Prepare reports for inspections, surveys, and internal audits.
• Manage record retention and destruction per state schedules and policies.
• Provide timely health information to the care team to support decisions.
• Participate in QAPI initiatives related to documentation and record accuracy.
• Maintain confidentiality and comply with privacy and security regulations.
