Job summary

AccessCommunity in Dearborn, MI seeks a Case Worker – Health Educator to provide specialized educational counseling within Community Health and Research Center services. Under general supervision, you will identify needs, support program access, and educate clients about services.

You will conduct outreach, coordinate care, translate for non-English speakers when needed, and meet education objectives. An Associate degree and 2+ years of relevant experience are required; Arabic language能力 is

Qualifications

Associate degree required in Business, Marketing, Public Health, Social Work or Human Services.
Two years of relevant experience in sales, marketing, public health, social work or human services preferred.
Diabetes program requires a certification as Diabetes Prevention Lifestyle Coach or to obtain within 3 months of hire.

Responsibilities

Provide advocacy and culturally appropriate health education to clients.
Provide care coordination, case management, and system navigation.
Build individual and community capacity by teaching clients how to manage their health needs.
Conduct outreach and home visits for education, assessment, and social support.
Document services per agency quality assurance plan and report progress per funding guidelines.
Create educational materials and program marketing content (newsletters, Facebook, websites).
May translate information for non-English speaking clients as needed.
