Job summary

accesscommunity seeks a Case Worker – Health Educator to provide education and advocacy within the Community Health and Research Center. You will coordinate care, navigate systems, and teach clients how to manage their health needs.

The role requires an Associate degree and 2+ years in relevant fields; experience with EMR systems and Microsoft Office is preferred. Some travel to community sites may be required, and bilingual skills are a plus.

Qualifications

• Associate degree required.
• Minimum 2 years of experience in sales, marketing, public health, social work, or human services.
• Certifications: Diabetes Prevention Lifestyle Coach preferred or to be obtained within 3 months; Chauffeur's license may be required after hire.

Responsibilities

• Provide advocacy, culturally appropriate health education and information to clients and communities.
• Care coordination, case management, and system navigation activities (ensuring access to care).
• Build individual and community capacity to manage health needs.
• Conduct assessments to identify needs and risks; perform outreach and home visiting; document services per QA plan.
• Prepare program progress reports and develop educational materials for newsletters, social media, and websites.
• Translate and interpret information for non-English speakers when needed; attend trainings and meetings as required.
