The Stormont Vail Health Command Center is a team working together to ensure that patients receive the right care, in the right setting, at the right time. Team members that work in the Command Center are responsible for health system capacity management including patient access and throughput, staffing resources and communication to internal and external stakeholders.

Qualifications

• High School Diploma / GED required or equivalent.
• Bachelor's degree preferred.
• 1 year healthcare experience preferred.

Responsabilités

• Assist House Supervisor / Patient Placement Coordinators with Capacity Management workflows.
• Coordinate patient transfers: collect condition, diagnosis, face-sheet and records.
• Serve as liaison for real-time bed status and updates to referring facilities.
• Communicate status updates with transferring facilities and clinical changes.
• Coordinate transportation for transfers between ED and inpatient areas.
• Assist with Out-of-Network transfer workflows.
• Facilitate provider-to-provider communications.
• Support staffing assignments with House Supervisor.
• Expedite patient flow through resource prioritization and communication.
• Process stat medical record requests and monitor dashboards for updates.
