Optimum Connect Health Services in Brooklyn, NY, is hiring an HCBS Supervisor to oversee daily HCBS program operations, supervise case managers and direct support staff, and ensure compliance with Medicaid and HCBS regulations.

This leadership role focuses on staff supervision, care coordination, documentation, quality assurance, and client advocacy, with in-person office work and community visits as required. A bachelor's degree and HCBS experience are required, with Master’s preferred.

Qualifications

• Bachelor’s degree in a relevant field is required; Master’s preferred.
• 2–3+ years of HCBS, Medicaid, or community-based services experience.
• Supervisory experience is preferred.
• Strong knowledge of Medicaid compliance and documentation requirements.
• Excellent leadership, communication, and organizational skills.

Responsibilities

• Supervise HCBS staff including case managers and direct support professionals.
• Assign and monitor caseloads to ensure timely service delivery.
• Ensure compliance with Medicaid and HCBS requirements.
• Oversee individualized service plans and person-centered care.
• Conduct staff training, supervision, and performance evaluations.
• Review documentation in EHR and care management systems.
• Serve as liaison for clients, families, and community partners.
• Participate in audits, quality improvement, and crisis response.
