Job summary

Lutheran Services Florida in Pensacola is seeking a dedicated Case Manager to coordinate guardianship services for up to 40 wards, arranging placement, monitoring care, and ensuring medical, social, psychiatric and financial needs are met.

The role requires a BA/BS in Social Services or related field, guardianship experience preferred, strong communication and organizational skills, and the ability to work independently with on-call field duties.

Qualifications

  • BA or BS in Social Services, Accounting, or LPN, RN required.
  • Experience in guardianship preferred.
  • Background in social services, medical nursing, legal, insurance or financial field required.

Responsibilities

  • Visit clients at least monthly and monitor care.
  • Prepare initial and annual court plans.
  • Maintain client files and case notes.
  • Assess client needs and coordinate services.
  • Arrange appropriate placement and monitor.
  • Document all client activity and advocate for clients.
  • Conduct case conferences with program staff and partners.
  • Interact with program volunteers and other professionals.
