Responsibilities include delivering PT services, documenting per CMS guidelines with NetHealth EMR, and collaborating with the interdisciplinary team. Requires active PT license and BS in PT, with ongoing CE to maintain licensure.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
• Must hold an active Physical Therapy license in state(s) of practice
• Ongoing continuing education to maintain licensure/certification
• Commitment to compliant documentation and professional development

Responsabilités

• Provide physical therapy services to residents per standards and productivity expectations
• Adhere to documentation and billing procedures
• Evaluate residents and develop treatment plans with short/long-term goals
• Conduct screenings to determine need for intervention
• Communicate findings and recommendations to residents, families, and interdisciplinary team
• Complete CMS-compliant documentation in NetHealth EMR
