Job summary

NPHire is seeking a Nurse Practitioner or Physician Assistant to provide specialized pain and palliative care to geriatric residents in skilled nursing facilities across the Cincinnati area. You'd round on the same residents, conduct pain assessments, create treatment plans, manage medications, and coordinate with facility staff and physicians.

Qualifications

Active, unrestricted Ohio NP or PA license.
Certification: AGACNP-BC, AGPCNP-BC, FNP-BC or PA-C.
Fully on-site availability and ability to travel between Cincinnati facilities.
Excellent communication, compassion and critical thinking.
Experience with geriatric care in SNFs is a plus.

Responsibilities

Conduct comprehensive pain assessments for geriatric residents.
Build individualized, evidence-based pain and palliative care plans.
Prescribe and manage pain medications per regulations.
Coordinate referrals to PT, acupuncture, blocks, and other interventional options.
Educate patients and families on pain management and lifestyle changes.
Collaborate with facility staff and physicians to coordinate care.
Monitor outcomes with follow-up visits and adjust plans as needed.
Maintain documentation in EHR systems (TEBRA, PointClickCare).
Develop strong relationships with leadership at partner facilities.
