Job description

A growing pain management practice is entering Ohio for the first time, and this Nurse Practitioner or Physician Assistant role is one of its first hires in the Cincinnati market. You'd provide specialized pain and palliative care to geriatric residents in skilled nursing facilities, rounding on the same patients over time rather than working through a rotating panel — conducting pain assessments, building individualized treatment plans, managing medications, and coordinating closely with facility staff and physicians.

Why You'll Love This Opportunity. A true ground-floor opportunity — you'd be shaping how this practice operates in a brand-new market, not stepping into someone else's established routine, with more positions opening as additional facilities come on board. Malpractice insurance with tail coverage included, protecting you against claims filed even after you leave the role — a benefit most employers don't offer, and exposure many providers otherwise carry quietly for years. New graduates are genuinely welcome, supported by a formal onboarding program rather than being thrown in cold; 5+ years of relevant RN experience also counts toward the experience bar. Deeply needed work — pain in older adults in skilled nursing facilities is chronically undertreated, and you'd be the provider who finally assesses and manages it properly, following the same residents week after week. Compensation that grows with your output: hourly base of $60–$100 depending on experience, plus a productivity bonus.

Key Responsibilities
  • Conduct comprehensive pain assessments, including medical history review, physical exams, and identifying the source and severity of pain.
  • Build individualized, evidence-based treatment plans combining pharmacological, non-pharmacological, and interventional approaches for acute and chronic pain.
  • Prescribe and manage pain medications, monitoring dosages in line with state and federal regulations.
  • Coordinate referrals for physical therapy, acupuncture, spinal blocks, and other interventional techniques.
  • Educate patients and families on pain management strategies, coping mechanisms, and lifestyle changes.
  • Collaborate with physicians, physical therapists, and other providers to keep care coordinated and outcomes on track.
  • Monitor treatment effectiveness through follow-up visits, adjusting plans as needed.
  • Maintain accurate, timely documentation in TEBRA and PointClickCare.
  • Build strong working relationships with leadership at each partner facility.
Required Qualifications
  • Active, unrestricted Ohio NP or PA license.
  • AGACNP- BC, AGPCNP-BC, FNP-BC or PA-C certification.
  • Fully on‑site availability — this role is not remote or hybrid.
  • Able to travel between facilities across the Cincinnati area.
  • Strong communication skills, compassion, and critical thinking, with a commitment to staying current in the field.
  • Ability to build and maintain professional relationships with facility leadership and staff.
Preferred Qualifications
  • Prior experience caring for geriatric patients.
  • Background in skilled nursing, long-term care, palliative care, or hospice.
  • Solid understanding of pain mechanisms, pharmacology, and the psychosocial aspects of chronic pain.
  • Experience with TEBRA or PointClickCare.
Compensation and Benefits
  • $60–$100/hour depending on experience, plus a productivity bonus.
  • Malpractice insurance, including tail coverage.
  • Formal onboarding program.
  • Full‑time hours, with schedule varying by facility and caseload.
  • Licensure costs is the candidate's responsibility; no sign‑on bonus or relocation assistance offered.
