Job summary

NPHire in Ohio is expanding its footprint by hiring two Nurse Practitioners or Physician Assistants to cover Columbus and Springfield, delivering specialized pain and palliative care to geriatric residents in skilled nursing facilities, with consistent follow-up for the same patients week after week.

You will conduct pain assessments, build individualized treatment plans, manage medications, and coordinate closely with facility staff and physicians, traveling between facilities as needed.

Qualifications

• Active, unrestricted Ohio NP or PA license is required.
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification.
• Fully onsite availability; travel between facilities is required.
• Experience caring for geriatric patients in skilled nursing settings preferred.

Responsibilities

• Conduct comprehensive pain assessments and medical history reviews.
• Develop individualized, evidence-based pain and palliative treatment plans.
• Prescribe and manage pain medications in line with regulations.
• Coordinate referrals for therapies and interventional procedures.
• Educate patients and families on pain management strategies.
• Collaborate with facility staff and physicians to ensure coordinated care.
• Document thoroughly in TEBRA and PointClickCare and track outcomes.
