Zusammenfassung

NPHire is expanding in the San Francisco area, seeking Nurse Practitioners or Physician Assistants to conduct comprehensive pain assessments in skilled nursing facilities. You will build individualized treatment plans, manage medications, and coordinate with facility staff weekly, focusing on geriatric pain—while traveling across multiple San Francisco facilities.

New graduates welcome with onboarding support; 5+ years RN experience counts toward requirements.

Qualifikationen

• Active, unrestricted California NP or PA license.
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification.
• Fully onsite availability; travel between facilities required.
• Ability to build relationships with facility staff and leadership.
• Willingness to stay current in geriatric pain management.

Aufgaben

• Conduct comprehensive pain assessments, including medical history, exams, and identifying pain source/severity.
• Build individualized, evidence-based treatment plans combining pharmacological, non-pharmacological, and interventional approaches.
• Prescribe and manage pain medications, monitoring dosages per state/federal regs.
• Coordinate referrals for physical therapy, acupuncture, spinal blocks, and other interventional techniques.
• Educate patients and families on pain management strategies and lifestyle changes.
• Collaborate with physicians, therapists, and other providers to keep care coordinated and outcomes on track.
• Monitor treatment effectiveness through follow-up visits and adjust plans as needed.
• Maintain accurate, timely documentation in TEBRA and PointClickCare.
• Build strong working relationships with leadership and staff at partner facilities.
