Iron Wellness and Recovery Center in Trenton, NJ is seeking a Care Coordinator for a grant-funded, full-time role focused on MOUD and coordinated care. You will support intake, enrollment, warm handoffs, and follow-up for clients receiving medication-assisted treatment, coordinating with clinical providers and community resources.

Competenze

• Bachelor's degree or higher in a related field is required.
• Experience in case management for SUD/behavioral health is preferred.
• Familiarity with ASAM Criteria and levels of care.

Mansioni

• Complete intake and screening with validated substance use and co-occurring mental health assessments.
• Support MOUD initiation and ensure timely admission within 72 hours.
• Provide retention-focused case management and follow-up.
• Coordinate with VA, housing, benefits, and community resources.
• Document all client contacts in the EHR and ensure confidentiality.
• Travel within Ocean County to deliver outreach and services.
