Iron Wellness and Recovery Center in Trenton, NJ seeks a Care Coordinator (Grant Position) to help expand MOUD access and guide clients through treatment and recovery services. This full-time role includes paid training, competitive compensation, and opportunities for advancement.

The role supports enrollment, intake, and ongoing care coordination, ensuring timely MOUD initiation and engagement in care, while collaborating with clinical providers and community resources.

Formación

• Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or related field.
• Experience providing case management in substance use or behavioral health.
• Familiarity with ASAM Criteria and levels of care.
• Willingness to obtain required credentials (e.g., CHW/PC).

Responsabilidades

• Support enrollment, intake, warm handoffs, retention, and follow-up for MOUD clients.
• Complete intake and screenings including validated substance use and mental health tests.
• Verify PDMP reviews for new admissions.
• Coordinate care with housing, benefits, employment, and VA resources.
• Maintain confidential client records and required data collection.
• Travel throughout the Ocean County service area.
