Job summary

City University Schools in Memphis is seeking a Transition Support Coordinator (10.5-month contract) to lead graduation success initiatives for high school scholars, with emphasis on Seniors, using data to identify risks and develop personalized plans that promote credit attainment and postsecondary readiness.

You will collaborate with teachers, counselors, parents, and community partners to deliver interventions, monitor progress quarterly, and coordinate college, military, and career

Qualifications

• Bachelor's degree in education or related content area.
• Master's degree preferred.
• Tennessee teaching/Service (Counseling, School Psychology, School Social Work) or Leadership certification preferred.
• 3–5 years of counseling, teaching, community engagement, leadership and/or partnership development experience required.

Responsibilities

• Supports all high school scholars, with priority on Seniors to avoid dropout while maintaining engagement.
• Develops and implements Individual Engagement and Career Plans for all scholars at risk of not graduating.
• Provides training to teachers on support measures for at-risk students, including attendance and tardiness issues.
• Develops credit recovery and differentiated instruction plans to meet needs of at-risk scholars while maintaining rigor.
• Collaborates with counselors, parents, and community partners to coordinate transition services and monitor progress.
