Job summary

City University School of Independence is seeking a Transition Support Coordinator to lead graduation planning and postsecondary readiness for scholars. You will coordinate data-driven interventions and collaborate with teachers, counselors and families to keep students on track for graduation and successful transitions to college, military, or workforce.

Qualifications

• What candidates must have: a Bachelor's degree in education, training, leadership, social sciences, curriculum or related area.
• Master's degree preferred; Tennessee teaching/leadership certificates preferred.
• 3–5 years of counseling, teaching, community engagement, leadership or program development experience.

Responsibilities

• Identify at-risk scholars and develop individualized support plans.
• Coordinate graduation, credit recovery and transition activities with staff and families.
• Analyze student data to inform interventions and measure progress.
• Collaborate with counselors, teachers and community partners to provide services.
