Job summary

City University School of Liberal Arts – Memphis, TN invites applications for the Transition Support Coordinator. This 10.5-month contract role reports to the Academic Director and focuses on graduation readiness, credit attainment, and postsecondary planning for scholars, with heavy emphasis on seniors.

You will work with faculty, counselors, families, and community partners to identify barriers and execute individualized support plans.

Qualifications

• Bachelor's degree in education, training, leadership, social sciences, curriculum or related content area.
• Master's degree preferred.
• 3-5 years of successful counseling, teaching, community engagement, leadership and program development experience required.

Responsibilities

• Supports all high school scholars, especially Seniors, to avoid the risk of dropping out and engage them in learning.
• Develops and implements Individual Engagement and Career Plans for all scholars, especially those at risk.
• Reviews and monitors Individual Engagement and Career Plans quarterly and makes adjustments as necessary.
• Provides training and support to teachers on support measures for at-risk students.
• Collaborates with counselors, parents and community partners to coordinate services and ensure students stay on track for graduation.
