City University School is seeking a Transition Support Coordinator, a 10.5-month, full-time role to drive graduation outcomes and postsecondary readiness. You will analyze scholar data, develop individualized plans, and coordinate with teachers, families, and community partners to remove barriers to graduation.

The position emphasizes seniors, credit recovery, and opportunities in college, military, and workforce transitions, while aligning with district goals and state requirements.

Qualifications

Bachelor's degree in education, training, leadership, social sciences, curriculum or related area.
Master's degree preferred.
3-5 years of successful counseling, teaching, community engagement, leadership and program development experience.

Responsibilities

Supports high school scholars with a priority on Seniors to avoid dropout risk.
Develops Individual Engagement and Career Plans and monitors progress quarterly.
Collaborates with teachers, counselors, parents and community partners to support transitions.
Provides or coordinates credit recovery, intervention strategies and postsecondary planning.
