Job summary

Fortis Care in Savannah, GA seeks a Nurse Practitioner or Physician Assistant to provide comprehensive care in a single skilled nursing facility. This Monday–Friday role has no night or weekend call and emphasizes high-quality, efficient, patient-centered care.

You will collaborate with the facility team to ensure timely, coordinated care and participate in quality initiatives to improve outcomes. Requires GA license and DEA, with 1–2 years' NP experience preferred.

Qualifications

• Current NP or PA license in Georgia.
• Unrestricted DEA license required.
• 1–2 years NP experience preferred; geriatrics/SNF exposure valued.

Responsibilities

• Provide direct medical care including assessment, diagnosis, and follow-up in a skilled nursing facility.
• Manage acute and chronic conditions with emphasis on quality and efficiency.
• Develop and update individualized care plans with the interdisciplinary team.
• Support transitions of care, including post-hospital evaluations and med reconciliation.
• Document encounters and care plans thoroughly and timely.
• Participate in quality initiatives to improve outcomes and efficiency.
• Collaborate with physicians and staff to ensure continuity of care.
