Job summary

Fortis Care in Marietta, GA is seeking a Nurse Practitioner to provide comprehensive clinical care in a skilled nursing facility. The role is Monday through Friday, with no night or weekend call, focusing on high-quality, patient-centered care and collaboration with physicians and the care team.

You will manage acute and chronic conditions, coordinate care plans with staff and families, ensure accurate documentation, and participate in quality initiatives to improve patient outcomes.

Qualifications

• Current NP license in Georgia.
• Certification in Internal Medicine, Geriatrics, or Family Practice required.
• Unrestricted DEA license.

Responsibilities

• Provide direct medical care to patients in the SNF, including assessment, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions with emphasis on high-quality, efficient care.
• Coordinate with facility staff, physicians, and families to develop/update individualized care plans.
• Support transitions of care, including post-hospital evaluations and medication reconciliation.
• Maintain thorough, timely documentation of patient encounters and care plans.
• Participate in quality initiatives and process improvements to enhance outcomes and efficiency.
• Collaborate with the interdisciplinary team to ensure continuity of care and adherence to evidence-based protocols.
