Job summary

Community LIFE seeks a full-time Primary Care Physician specialized in geriatrics for its McKeesport clinic. The physician will perform periodic and episodic assessments, coordinate care with specialists, and oversee inpatient needs within an interdisciplinary team.

Expect on-site clinic work with occasional home or SNF visits. Requirements include MD/DO, PA license, DEA, Medicare enrollment, and a background in geriatrics; residency in Internal Medicine or Family Medicine is required, with

Qualifications

• Graduate of an accredited medical school.
• Residency in Internal Medicine or Family Medicine completed.
• Geriatrics fellowship preferred.
• Current Pennsylvania MD or DO license; active DEA; Medicare enrollment.

Responsibilities

• Periodically assess and manage medical conditions of frail elderly participants.
• Oversee use of medical specialists and inpatient care.
• See 4–6 patients per day in an on-site clinic; occasional home or SNF visits.
• Collaborate with an Interdisciplinary Team to plan and provide care.
