Job summary

TEEMA Solutions Group is seeking a Physician to provide direct primary medical care to patients and supervise medical care personnel, including PCP Nurse Practitioners, RNs and Certified Medical Assistants, within an Interdisciplinary Team.

The role includes performing comprehensive history and physicals on new referrals, coordinating hospital admissions, and managing nursing home care, while actively participating in care planning, ethics, and quality improvement initiatives to uphold geriatric

Qualifications

  • Valid MD/DO license in good standing to practice medicine in the State.
  • Graduate of an accredited School of Medicine.
  • Board certification.
  • Minimum of two years of clinical experience.
  • Experience or training in working with frail or elderly populations.
  • Knowledge of geriatric medicine and the special needs of complex geriatric patients.
  • Medically cleared for communicable diseases and up-to-date immunizations.
  • Ability to pass a criminal background check and Medicare/Medicaid eligibility verification.

Responsibilities

  • Provide direct primary medical care to patients within the Interdisciplinary Team framework.
  • Perform comprehensive history and physical examinations on new referrals.
  • Conduct interval history and physicals on established patients quarterly or as indicated.
  • Evaluate and treat patients with episodic illnesses.
  • Refer patients to medical specialists as needed.
  • Coordinate hospital admissions and discharge planning with the IDT.
  • Manage care for nursing home residents, including regular visits and communication with staff.
  • Participate in family conferences for care planning.
  • Document patient changes in medical records and communicate updates to the IDT.
  • Attend staff meetings and contribute to care planning as a member of the IDT.
  • Maintain and enhance education regarding quality clinical standards and best practices for geriatric care.
  • Serve on Ethics and Quality Improvement committees or work groups as assigned.

Overview

Our client is seeking a Physician to provide direct primary medical care to patients and supervise medical care personnel, including PCP Nurse Practitioners, RNs, and Certified Medical Assistants. The Physician will deliver care within the Interdisciplinary Team framework.

Perform comprehensive history and physical examinations on new referrals.

Conduct interval history and physicals on established patients quarterly or as indicated.

Evaluate and treat patients with episodic illnesses.

Refer patients to medical specialists as needed.

Coordinate hospital admissions and discharge planning with the IDT.

Manage care for patients residing in nursing homes, including regular visits and communication with nursing home staff.

Actively participate in family conferences for care planning.

Document patient changes in medical records and communicate updates to the IDT.

Participate as a member of the IDT, attending staff meetings and contributing to care planning.

Maintain and enhance education regarding quality clinical standards and best practices for geriatric care.

Serve on Ethics and Quality Improvement Committees or work groups as assigned.

Required Qualifications

Valid MD/DO license in good standing to practice medicine in the State practice.
