Job summary

Community LIFE is seeking a Primary Care Physician in McKeesport, PA to provide geriatric primary care in an on-site clinic with occasional home/SNF visits. The role requires a medical degree and PA licensure, with residency in Internal Medicine or Family Medicine; geriatrics fellowship preferred.

The physician will be part of an Interdisciplinary Team, managing 4–6 patients daily and coordinating with specialists and inpatient services. Relocation support may be available.

Qualifications

• Graduate of an accredited medical school with MD or DO.
• Residency in Internal Medicine or Family Medicine.
• Fellowship training in geriatrics preferred.
• Current PA medical license and active PA DEA license.
• Medicare provider enrollment.
• Valid PA driver's license.
• Minimum one year of primary care experience with frail or elderly patients.

Responsibilities

• Periodic and episodic assessment and management of participants' medical conditions.
• Oversee use of medical specialists and inpatient care.
• See 4–6 patients per day in on-site clinic; occasional home or SNF visits.
• Work as part of an Interdisciplinary Team to plan and provide care.
