Job summary

Senior LIFE is seeking an accomplished physician to provide comprehensive primary care for seniors at our Allentown center. The role involves leading interdisciplinary teams, coordinating care across hospital and community settings, and delivering patient-centered geriatric care.

The ideal candidate will be licensed in Pennsylvania, with geriatrics fellowship preferred and at least one year of experience with frail elderly patients. A commitment to teamwork and patient advocacy is essential.

Qualifications

• Licensed to practice medicine in Pennsylvania.
• Fellowship in geriatrics preferred.
• Minimum of one year experience with frail/elderly populations.
• Proven clinical leadership and teamwork in fast-paced settings.

Responsibilities

• Perform comprehensive histories and physicals for new referrals to determine best care plans.
• Address diverse diagnoses with interdisciplinary teams including social workers and therapists.
• Coordinate discharge planning and follow-up in collaboration with LIFE Center staff.
• Refer to specialists as needed and support hospital admissions when necessary.
• Communicate treatment plans clearly with members, families, and caregivers.
