Job summary

Community LIFE is seeking a Primary Care Physician in McKeesport, PA to provide geriatric primary care in an on-site clinic with occasional home/SNF visits. The role requires a medical degree and PA licensure, with residency in Internal Medicine or Family Medicine; geriatrics fellowship preferred.

The physician will be part of an Interdisciplinary Team, managing 4–6 patients daily and coordinating with specialists and inpatient services. Relocation support may be available.

Qualifications

• Graduate of an accredited medical school with MD or DO.
• Residency in Internal Medicine or Family Medicine.
• Fellowship training in geriatrics preferred.
• Current PA medical license and active PA DEA license.
• Medicare provider enrollment.
• Valid PA driver's license.
• Minimum one year of primary care experience with frail or elderly patients.

Responsibilities

Shift: Mon-Fri, 7:30am-4:00pm plus limited on-call

Location: McKeesport, PA 15132- Relocation assistance may be available

Summary of Job Duties: Our Primary Care Physician is responsible for periodic and episodic assessment and management of individual participants' medical conditions, and for oversight of their use of medical specialists and inpatient care. Our Physician typically sees 4-6 patients per day. Patients are seen in our on-site clinic, plus occasional visits to homes or SNFs. The Physician is part of an Interdisciplinary Team who works together to assess, plan care for, and provide service to frail, elderly participants.

Required Education: Graduate of an accredited school of medicine. Completion of residency in Internal Medicine or Family Medicine. Fellowship training in geriatrics preferred.

Required Licensure:Must have a current license to practice medicine in Pennsylvania (MD or DO), current DEA license, Medicare provider enrollment, and a valid PA driver's license.

Required Experience:A minimum of one year of primary care experience with a frail or elderly population.

Required Skills:

• Geriatric Assessment and management of co-morbidity.
• Effective listening and communication skills with patients, families and IDT members.
• Clear and complete documentation.
• Effective Team skills including but not limited to reliability, active listening, positive approaches, courtesy and respect in line with Community LIFE Mission and Values statements.
