Job summary

Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide care for high-risk patients in assisted living and senior living communities in the Salem area. This role blends geriatrics, internal medicine, chronic disease management, and palliative care within a value-based care model.

The position offers a flexible schedule with a minimum commitment of 24 hours per month, on-call responsibilities, and an emphasis on quality patient care over visit volume.

Qualifications

• Active state license (or ability to obtain)
• Active DEA license or ability to obtain
• New graduates encouraged to apply
• Experience in geriatrics or senior living preferred
• Strong documentation and EMR skills

Responsibilities

• Conduct comprehensive patient assessments
• Manage acute illnesses and chronic disease exacerbations
• Coordinate testing, diagnostics, and treatment interventions
• Ensure accurate documentation and ICD-10 coding
• Collaborate with interdisciplinary care teams
• Conduct advance care planning discussions
• Provide education and counseling to patients and caregivers
• Coordinate care transitions and develop individualized care plans
