Job summary

Optum in Salem, OR seeks a Nurse Practitioner or Physician Assistant to deliver high-risk patient care in assisted living and senior communities. This role blends geriatrics, internal medicine, chronic disease management, and palliative care within a value-based model.

You will conduct assessments, manage illnesses, coordinate tests and treatments, and work with an interdisciplinary team to support patients and caregivers across transitions.

Qualifications

• MSN or DNP with AANP/ANCC certification for NP track.
• PA track requires NCCPA certification and relevant program completion.
• Active state license or ability to obtain license; DEA license or ability to obtain.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute illnesses and chronic disease exacerbations.
• Coordinate testing, diagnostics, and treatment interventions.
• Ensure accurate documentation and ICD-10 coding.
• Collaborate with interdisciplinary care teams.
• Conduct advanced illness and advance care planning discussions.
• Provide education and counseling to patients and caregivers.
• Coordinate care transitions and develop individualized care plans.
