Job summary

Optum Senior Community Care is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care for high-risk patients in nursing homes, assisted living communities, and senior housing throughout Oregon.

The role features a value-based care model with no productivity quotas, no on-call, no weekends, and a Monday–Friday schedule. You will collaborate with an interdisciplinary team to improve outcomes and deliver patient-centered care in diverse settings.

Qualifications

• MSN or DNP with NP/PA certification.
• Active Oregon license or ability to obtain.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute illnesses and chronic disease exacerbations.
• Support patients during short-term rehabilitation stays.
• Order and interpret testing and interventions as appropriate.
• Ensure accurate documentation and ICD-10 coding.
• Collaborate with interdisciplinary care teams.
• Conduct advance care planning discussions.
• Educate patients and caregivers regarding treatment plans and goals.
