Job summary

Optum Senior Community Care (SCC) seeks a Nurse Practitioner or Physician Assistant to provide longitudinal care for high‑risk patients in nursing homes, assisted living, and senior housing across Washington and Multnomah Counties, Oregon.

Join a value‑based care model with no productivity requirements, a regular weekday schedule, no on‑call, and strong interdisciplinary teamwork to improve outcomes for medically complex geriatric patients while supporting their goals of care.

Qualifications

• MSN or DNP with AANP/ANCC certification for NP, or NCCPA certification for PA.
• Active Oregon license or ability to obtain; DEA license or ability to obtain.
• Ability to travel between facilities and perform comprehensive assessments.

Responsibilities

• Conduct comprehensive patient assessments.
• Manage acute illnesses and chronic disease exacerbations.
• Support patients during short-term rehabilitation stays.
• Order and interpret testing, diagnostics, and interventions as appropriate.
• Ensure accurate documentation and ICD-10 coding.
• Collaborate with interdisciplinary care teams.
• Discuss advance care planning with patients and families.
• Educate patients and caregivers regarding treatment plans and goals.
