Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with emphasis in primary care or internal medicine, ideally geriatrics-focused, to join our team serving residents in long-term care facilities and assisted living communities. You will collaborate with physicians, RN case managers, and health plan leadership to implement our national care model and improve quality of life for seniors.

You will exercise independent judgment in treating acute and chronic conditions, with supervision as

Qualifications

FNP, AGNP, AHACNP or PA license required
Degree from an accredited APRN or PA program
3 years’ experience in clinical nursing or rehab in geriatric populations
Electronic Health Records experience
Working knowledge of Microsoft applications, including Word, Outlook and Excel

Responsibilities

Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care
Ensure compliance with local, state, and federal agencies related to clinical services you provide
Prescribe medications and order lab work, diagnostic procedures and consultations
Monitor patients’ compliance and response to their treatment and modify those plans
Work with RN case managers as part of an integrated care team
