Job summary

American Health Partners’ TruHealth division seeks an advanced practice provider (NP or PA) with a geriatrics focus to join our team serving nursing homes and assisted living communities. You will deliver personalized, coordinated care and collaborate with physicians, case managers, and facility staff.

You will assess medical and mental health needs, prescribe medications, order labs, and ensure compliance with federal and state regulations while continuously working to improve patient outcomes

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in geriatric clinical nursing or rehab.

Responsibilities

• Assess patients’ medical and mental health needs and create a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ response to treatment and modify plans as needed.
