Job summary
TruHealth, part of American Health Partners, seeks an advanced practice provider (NP/PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a multidisciplinary team to deliver coordinated, patient-centered care within nursing homes and assisted living communities enrolled in Medicare Advantage plans.
You will exercise independent judgment in treating acute and chronic conditions, consulting a supervising physician as appropriate, and collaborating with
Qualifications
- Masters degree or higher in nursing or related field
- APRN or PA license with current state authorization
- 3 years of clinical nursing or rehab experience in geriatric populations
- Proficient with Electronic Health Records
- Strong computer skills including Word, Outlook, Excel
Responsibilities
- Assess patients' medical and mental health needs and design proactive plans of care
- Ensure compliance with local, state, and federal regulations governing clinical services
- Prescribe medications and order lab tests, diagnostics, and consultations
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with RN case managers and an integrated care team
