Job summary

American Health Partners' TruHealth division seeks an advanced practice provider (NP or PA) with geriatrics experience to join our team serving residents in nursing homes and assisted living communities. You will deliver personalized, coordinated care and work with a multidisciplinary team to implement Treat in Place protocols in compliance with regulations.

The ideal candidate holds APRN or PA licensure and has 3+ years in geriatric clinical care with EHR proficiency.

Qualifications

• APRN or PA license required.
• 3 years’ experience in geriatric clinical nursing or rehab.
• Electronic Health Records experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assessing and treating patients in nursing homes or assisted living facilities.
• Coordinate care with supervising physician and care teams.
• Prescribe medications and order labs and diagnostics as appropriate.
• Ensure compliance with local, state, and federal regulations.
