Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. You will work with a team of providers and RN case managers to deliver personalized, coordinated care that improves residents' quality of life and reduces hospital

Qualifications

• Masters degree or higher in nursing or related field.
• APRN/PA license required to practice.
• 3 years’ experience in geriatric clinical nursing or rehab.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, tests and referrals.
• Monitor patient progress and adjust treatment plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
