Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to join our team serving residents in long-term care. The role focuses on delivering personalized, coordinated health care in nursing homes or assisted living communities to improve quality of life.

You will exercise independent judgment, consult with a supervising physician as needed, and work closely with facility staff and health plan leadership to implement our national care model in compliance with

Qualifications

Masters degree or higher.
Degree from an accredited APRN or PA program.
3 years of clinical nursing or rehab experience in geriatric populations.
Experience with Electronic Health Records (EHR).
Microsoft Office proficiency (Word/Outlook/Excel).

Responsibilities

Assess patients’ medical and mental health needs and develop proactive care plans.
Ensure compliance with local, state, and federal regulations.
Prescribe medications and order labs, diagnostic procedures, and consultations.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with RN case managers as part of an integrated care team.
