Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric or primary care focus to improve outcomes for long-term care residents. You’ll work with a team of providers and case managers in nursing homes or assisted living communities enrolled in Medicare Advantage plans, applying independent judgement with physician supervision as needed.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ response and modify treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
