Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. You will join a team delivering coordinated care to residents in nursing homes and assisted living communities enrolled in Medicare Advantage plans, improving outcomes and quality of life.

Qualifications

• APRN or PA license required.
• Masters degree or higher in nursing or a related field.
• Minimum 3 years of geriatric clinical nursing or rehab experience.

Responsibilities

• Assess residents' medical and mental health needs and create proactive care plans.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications, order labs, and coordinate consultations.
• Monitor treatment responses and adjust plans as needed.
• Collaborate with RN case managers within an integrated care team.
