TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care within long-term care and assisted living settings. You will work closely with a team to provide patient-centered, coordinated care that improves outcomes and reduces readmissions.

The role emphasizes independent clinical judgment, collaboration with supervising physicians, and adherence to Treat in Place protocols while adhering to federal and state regulations.

Formación

• APRN or PA license required
• 3 years’ experience in geriatric nursing or rehab
• Experience with Fee for Service model is a plus

Responsabilidades

• Assess medical and mental health needs and create a care plan
• Ensure compliance with local, state, and federal regulations
• Prescribe medications and order labs, imaging, and consults
• Monitor patient progress and adjust treatment plans as needed
