Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with a geriatrics focus to join our team. You will work primarily in nursing homes or assisted living communities, delivering personalized, coordinated healthcare and improving resident outcomes.

You’ll exercise independent judgment, consult with a supervising physician, and collaborate with facility partners and health plan leadership to implement Treat in Place Protocols in

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Proficient with Microsoft Word, Outlook, and Excel.

Responsibilities

• Assessing patients’ medical and mental health needs and providing a plan of care
• Ensuring compliance with local, state, and federal agencies related to clinical services you provide
• Prescribing medications and ordering lab work, diagnostic procedures and consultations
• Monitoring patients’ compliance and response to treatment and modifying plans
