Zusammenfassung

TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care to residents in nursing homes or assisted living communities. You will exercise independent judgment to treat acute and chronic conditions, collaborating with a supervising physician as appropriate.

Join TruHealth, a division of American Health Partners, and work with facility partners to implement Treat in Place Protocols while ensuring compliance with regulations.

Qualifikationen

• APRN or PA license is required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Aufgaben

• Assessing patients’ medical and mental health needs and providing a plan of care.
• Ensuring compliance with local, state, and federal agencies related to clinical services.
• Prescribing medications and ordering lab work, diagnostic procedures and consultations.
• Monitoring patients’ compliance and response to their treatment and modifying those plans.
