Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with a geriatrics focus to join our team. Our providers work primarily within nursing homes or assisted living communities, delivering personalized, coordinated health care that improves patient outcomes and reduces re-hospitalizations.

You will exercise independent clinical judgment and partner with supervising physicians as appropriate. This Fee For Service role includes a competitive pay package and a $5,000 sign-on bonus.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Prescribe medications and order labs, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and adjust plans accordingly.
• Ensure compliance with local, state, and federal regulations related to clinical services.
