Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with experience in geriatrics to join our team. Our providers work primarily within nursing homes or assisted living communities, delivering personalized, coordinated care and exercising independent professional judgement with supervisory physician support.

You will implement Treat in Place Protocols, collaborate with facility partners and health plan leadership, and contribute to improving resident outcomes while adhering to all

Qualifications

• 3 years’ experience in clinical nursing or rehab in geriatric populations or Fee for Service exposure.
• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• EHR experience and MS Office proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and develop a care plan.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications, order labs, and coordinate consultations.
• Monitor patient progress and adjust treatment plans as needed.
