Zusammenfassung

TruHealth is seeking an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics-focused. You will improve outcomes for long-term care residents by collaborating with a team of providers and case managers, supported by a strong benefits and compensation package.

You will work with nursing homes and assisted living communities, implementing Treat in Place Protocols in alignment with regulations.

Qualifikationen

• APRN/PA license required.
• Degree from an APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations, or fee-for-service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Aufgaben

• Assess patients’ medical and mental health needs and plan care to proactively manage conditions.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostic procedures and consultations.
• Monitor patient progress and adjust treatment plans as needed.
