Job summary

TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care within nursing homes or assisted living communities. You will collaborate with facility partners and case managers to improve outcomes and reduce hospitalizations while enjoying a competitive compensation package.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patient progress and adapt treatment plans as needed.
