Job summary

TruHealth is seeking an advanced practice provider (NP or PA) to serve aging populations in nursing homes or assisted living facilities. You will exercise independent judgment to treat acute and chronic conditions, coordinating with supervising physicians as needed.

You will collaborate with facility partners and health plan leadership to implement Treat in Place Protocols in full compliance with federal and state rules.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop a plan of care that proactively manages conditions.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and modify plans accordingly.
