Job summary

Rocky Mountain Health Care Services (RMHCS) seeks a Nurse Practitioner to join its PACE IDT, delivering wraparound, person-centered care to older adults with chronic illness and frailty. You will work with PCPs, nurses, social workers, rehab therapists, and other specialists in clinic, home, and community settings.

This role emphasizes collaboration, quality improvement, and mentorship within a compassionate, mission-driven nonprofit.

Qualifications

• Current NP licensure (AGNP, FNP, GNP) and prescriptive authority in Colorado.
• Experience in geriatrics or family/internal medicine with interest in palliative care.
• Strong clinical skills and commitment to interdisciplinary collaboration.
• Excellent communicator and advocate for vulnerable populations.
• Commitment to shared learning, self-care, and care equity.

Responsibilities

• Provide whole-person primary and supportive care in the clinic, home, and community-based settings.
• Partner with participants and families in managing chronic and serious illness, goals of care, and care planning.
• Co-lead IDT discussions, contributing your voice to shared decision-making.
• Participate in quality improvement and professional development initiatives guided by physician mentors and coaches.
• Foster trust, presence, and accountability across all disciplines.
